Your Choice Support Coordination (YCSC) is expanding, and we are looking for a highly capable, passionate Support Coordinator to join our dedicated team. We are a fully remote organization committed to empowering NDIS participants to achieve their goals and maximize their plans.
While the majority of our participant base is located in the Townsville, QLD area, we also support a growing number of participants in Perth, WA. Because our entire team operates remotely, we welcome applicants from across the country. We are less concerned about your physical location and entirely focused on your expertise, drive, and ability to hit the ground running from day one.
The Role Stepping into this role, you will take ownership of a diverse caseload, acting as a vital link between participants and their support networks. We are looking for a self-starter who thrives in an autonomous environment and brings a deep understanding of the NDIS landscape.
Key Responsibilities
- Provide high-level, end-to-end Support Coordination, guiding participants through the implementation of their NDIS plans.
- Deliver robust case management and seamlessly coordinate services across allied health, community providers, and mainstream supports.
- Maintain meticulous, compliant, and timely case notes and documentation using our designated CRM platform, Astalty.
- Work highly autonomously to manage competing priorities, budgets, and reporting deadlines.
- Contribute your passion and insights to our team culture as we continue to scale and refine our service delivery.
What You Bring
- Proven Experience: Demonstrated, hands-on experience as a Support Coordinator. You must be able to step into the role and manage a caseload immediately.
- NDIS Expertise: A comprehensive, practical knowledge of the NDIS framework, current pricing arrangements, and operational guidelines.
- Administrative Excellence: Exceptional attention to detail with a strong track record of maintaining accurate documentation and compliance records.
- Independent Drive: The proven ability to work autonomously and effectively in a 100% remote working environment.
- Highly Desirable: Formal qualifications in social work, psychology, disability services, or a closely related human services field.
