About the Role
We are seeking motivated, experienced and participant-focused NDIS Support Coordinators and Specialist Support Coordinators to join our team.
In this role, you will support NDIS participants to understand and implement their plans, connect with appropriate service providers, manage their supports and work towards their individual goals.
Depending on your experience and qualifications, you may work at Level 2 Support Coordination or Level 3 Specialist Support Coordination, supporting participants with more complex needs, risks and barriers.
Specialist Support Coordinators will be responsible for coordinating complex support arrangements, managing service-related risks and working collaboratively with multidisciplinary teams to improve participant outcomes.
Weekly hours are not fixed and will depend on participant demand, caseload complexity, billable hours and achievement of agreed key performance indicators (KPIs).
Key Responsibilities
- Support participants to understand, implement and make effective use of their NDIS plans.
- Develop, coordinate and review person-centred support arrangements.
- Connect participants with suitable disability providers, allied health professionals and community services.
- Monitor participant funding, service agreements and plan utilisation.
- Liaise with participants, families, support networks, plan managers, clinicians, government agencies and service providers.
- Complete participant intake documentation, case notes, progress reports and other required records.
- Facilitate meetings, case conferences and plan reviews where required.
- Identify barriers to service access and help participants navigate challenges.
- For Specialist Support Coordination, manage complex circumstances, service breakdowns, significant risks and stakeholder concerns.
- Develop and review complex support arrangements and coordinate multidisciplinary services where required.
- Maintain accurate documentation and ensure compliance with relevant NDIS requirements.
- Develop referral networks and contribute to participant-base growth.
- Meet agreed KPIs relating to participant onboarding, caseload development, billable hours, reporting, risk management, service quality and participant retention.
Essential Requirements
- Minimum two years of relevant experience in Support Coordination, Specialist Support Coordination or a related disability, health or community services role.
- Demonstrated understanding of the NDIS, participant plans and relevant regulatory requirements.
- Strong communication, organisation, assessment and stakeholder-management skills.
- Ability to manage a caseload independently and work effectively in a hybrid environment.
- Existing disability-sector connections and the ability to develop professional referral relationships.
- Strong written communication and report-writing skills.
- Valid driver's licence and access to a reliable, appropriately insured vehicle.
- Current NDIS Worker Screening Check and other required clearances.
- Current First Aid and CPR certificates.
- Australian working rights.
Additional requirements for Level 3 Specialist Support Coordination:
- Demonstrated experience supporting participants with complex needs, significant risks or multiple service barriers.
- Experience coordinating multidisciplinary supports and managing complex service arrangements.
- Relevant qualification in social work, psychology, allied health, community services or a related discipline.
Remuneration Hourly rates will depend on the level of the role and the successful candidate's relevant experience:
- Level 2 – NDIS Support Coordinator: $65 per hour (casual).
- Level 3 – NDIS Specialist Support Coordinator: $80 per hour (casual).
Hours and KPI Expectations Weekly hours are flexible and will depend on the active caseload, participant demand, caseload complexity, billable hours and achievement of agreed performance expectations.
KPIs may include
- Participant onboarding and caseload growth.
- Billable Support Coordination hours.
- Timely and accurate documentation and reporting.
- Participant engagement, retention and service quality.
- Effective risk identification and management, particularly for complex participants.
- Development and maintenance of referral networks.
- Compliance with relevant NDIS requirements and organisational procedures.
