Registered Nurse (RN) – Clinical Care Manager | Aged Care & Support at Home
Organisation: CarePro
Location: South East Melbourne, VIC
Employment Type: Negotiable – Part-time or Full-time
Experience: Extensive aged care clinical management experience essential
About CarePro
CarePro is an established registered NDIS provider looking to expand its services into aged care. We are seeking an experienced and highly capable Registered Nurse (RN) to play a key role in our aged care registration, clinical governance and audit preparation, followed by a long-term leadership role overseeing clinical care across our growing aged care services.
This is an opportunity to help establish and shape CarePro’s aged care clinical systems from the ground up, ensuring our policies, procedures, assessments, care planning and clinical governance arrangements meet applicable aged care regulatory and quality standards.
Following the registration and establishment phase, the successful candidate will take on an ongoing clinical leadership role, primarily supporting CarePro’s aged care business, with a strong focus on delivering safe, high-quality care to older people living in their own homes under the Support at Home (SAH) program, formerly Home Care Packages (HCP), alongside relevant Commonwealth Home Support Programme (CHSP) services where applicable.
About the Role The Clinical Care Manager is the paramount clinical authority at CarePro. The position is strictly separated from operational rostering pressures to ensure that clinical safety, quality of care and participant wellbeing remain the priority.
The successful candidate will initially work closely with management to prepare CarePro for aged care registration, implementation and audit requirements. This will include developing and reviewing clinical governance frameworks, establishing clinical systems, preparing evidence and documentation, and ensuring clinical processes are embedded into everyday practice.
Once the aged care service is established, the role will transition into an ongoing clinical management position responsible for clinical assessments, care planning, staff training, clinical oversight and the management of complex clinical risks.
We are specifically looking for a nurse with substantial experience providing and managing community-based aged care services, supporting older people in their own homes, rather than someone whose experience is limited to residential aged care.
Key Responsibilities1. Aged Care Registration & Audit Preparation

- Assist in developing and implementing clinical governance frameworks, policies, procedures and clinical documentation.
- Establish systems aligned with applicable Aged Care Quality Standards, the Aged Care Act and relevant regulatory requirements.
- Prepare clinical evidence, records and documentation for registration assessments, audits and regulatory reviews.
- Identify gaps in existing systems, develop corrective action plans and monitor implementation.

- Assess clinical needs relating to mobility, falls, nutrition, hydration, continence, skin integrity, medication management, cognition and other relevant health concerns.
- Identify deterioration, changing support needs and circumstances requiring escalation to medical practitioners or other health professionals.
- Coordinate care with GPs, allied health professionals, families, carers and other relevant providers.
- Ensure clinical assessments and care plans are accurate, current and appropriately documented.
1. Clinical Governance & Risk Management
- Act as CarePro’s senior clinical authority, providing independent clinical direction and oversight.
- Lead the response to high-impact and high-prevalence clinical risks.
- Establish and monitor clinical indicators, quality reviews and improvement initiatives.
- Oversee incident reviews, clinical investigations, corrective actions and escalation pathways.
- Ensure clinical decisions are based on skilled judgement, evidence-based practice and the individual’s assessed needs.
- Maintain clear separation between clinical decision-making and operational or rostering pressures.

- Train, guide and mentor support workers and other relevant staff in safe care practices.
- Establish clinical competency requirements and assess staff capability where appropriate.
- Provide education in areas such as personal care, infection prevention, medication support, falls prevention, recognising deterioration and incident escalation.
- Develop and oversee clinical delegation arrangements within professional scope, applicable legislation and organisational policies.
- Ensure staff understand individual care plans, identified risks and escalation procedures.
- Provide ongoing clinical supervision and support to the workforce.
1. Ongoing Aged Care Clinical Management
- Provide continuing clinical oversight of CarePro’s aged care services, with a primary focus on community-based care.
- Review changes in clients’ health, functional ability and support requirements.
- Support safe service delivery and appropriate coordination of clinical and non-clinical services.
- Contribute to service development, quality improvement and the expansion of CarePro’s aged care operations.
- Maintain accurate clinical records and ensure appropriate follow-up of identified risks and care needs.
