HomeCare Registered Nurse - Melbourne VIC (Western Suburbs, South Eastern Suburbs, Eastern Suburbs, Northern Suburbs)

We are seeking a compassionate and autonomous Registered Nurse to join our Community & Home Care team. In this role, you will provide high-quality nursing care to clients in their homes, supporting independence, wellbeing, and safety. You will work closely with clients, families, and multidisciplinary teams to deliver person-centred care that aligns with AHPRA and NMBA standards.

Key Responsibilities

- Conduct comprehensive in-home assessments and develop personalised care plans.
- Provide wound care, medication administration, catheter care, and other clinical interventions.
- Monitor client health, identify early signs of deterioration, and escalate as required.
- Coordinate care with GPs, allied health professionals, and support workers.
- Educate clients and families on health management, chronic disease support, and self-care strategies.
- Maintain accurate documentation, progress notes, and clinical records.




- Support care workers with clinical advice and delegation within their scope of practice.
- Promote client independence, dignity, and safety in the home setting.

- Empathetic, patient-centred approach with cultural awareness.
- Comfortable travelling between client homes (travel allowance provided).

Qualifications & Requirements

- Bachelor of Nursing (or equivalent).
- Current AHPRA registration as a Registered Nurse.
- Current CPR/Basic Life Support (BLS) certification.
- Valid driver’s licence and reliable vehicle.
- National Police Check and/or NDIS Worker Screening Check (as applicable).
- Up-to-date vaccinations per state/territory health requirements.

Pay: $40.00 – $68.81 per hour
