Support Coordinator – NDIS
Location: Point Cook VIC
Employment Type: Casual (Hybrid)
Salary: $60000-100000 annual
About the Role
We are seeking a motivated, compassionate, and organized Support Coordinator to join our team. In this role, you will support NDIS participants to understand and implement their NDIS plans, connect with appropriate services, and work towards achieving their personal goals, independence, and improved quality of life.
The successful candidate will have excellent communication and organizational skills, a person-centred approach, and the ability to build strong relationships with participants, families, carers, and service providers.
Key Responsibilities
- Coordinate supports and services in line with participants’ NDIS plans and individual goals.
- Assist participants to understand their plans, budgets, and available support options.
- Connect participants with suitable disability, health, community, and mainstream services.
- Build and maintain positive relationships with participants, families, carers, and service providers.
- Monitor participants’ progress and review support arrangements to ensure their needs are being met.
- Identify barriers to accessing services and assist participants to develop practical solutions.
- Support participants to exercise choice and control over their services and achieve greater independence.
- Maintain accurate participant records, case notes, reports, and documentation.
- Liaise with service providers and other stakeholders to ensure coordinated service delivery.
- Identify and respond appropriately to risks, safeguarding concerns,
and changes in participants’ circumstances.
- Ensure all services are delivered in accordance with relevant NDIS requirements, organizational policies, and applicable legislation.
Essential Criteria
- Experience in support coordination, disability services, community services, or a related field.
- Understanding of the NDIS framework, participant plans, and service delivery processes.
- Strong communication, interpersonal, problem-solving, and time-management skills.
- Ability to manage competing priorities and work independently.
- Excellent record-keeping, documentation, and administrative skills.
- A genuine commitment to person-centred practice, participant choice, and individual empowerment.
- Ability to build skilled relationships with participants and a range of stakeholders.
- Current Australian driver's licence and willingness to travel, where required.
- Relevant qualifications in Disability, Community Services, Social Work, or a related discipline will be highly regarded.
Desirable
- Previous experience working with NDIS participants and coordinating complex support needs.
- Knowledge of NDIS funding categories, service agreements, and referral pathways.
- Experience supporting participants with diverse needs and backgrounds.
- Current NDIS Worker Screening Check and Working with Children Check, where applicable.
