About the Role

Wise Healthcare is seeking a reliable, compassionate and enthusiastic NDIS Support Worker to provide community access support to a participant located in West End, QLD.

The successful candidate will assist the participant in accessing the community, attending appointments, participating in social activities and promoting independence and confidence in everyday life.

Available Shifts

- Monday: 4.5 hours
- Friday: 4.5 hours
- Total: 9 hours per week
- Ongoing shifts available

Key Responsibilities

- Support the participant with community access and social outings.
- Assist with attending appointments, shopping and recreational activities.
- Provide transport to community activities as required.
- Encourage independence, confidence and social participation.
- Promote participant choice, dignity and wellbeing.
- Maintain accurate shift notes and communicate with the care team.

Requirements

- Previous experience in disability support or community care preferred.
- Current NDIS Worker Screening Check or willingness to obtain one.




- Current First Aid and CPR certificates.
- Valid driver’s licence and access to a reliable, insured vehicle.
- National Police Check.
- Robust communication and interpersonal skills.
- Reliable, punctual and committed to providing quality support.
