Home Caring Tranmere is looking for a motivated, compassionate and capable NDIS Service Team Leader to join our growing team.

As our NDIS Service Team Leader, you will play a key role in overseeing the day-to-day operations of 24/7 supported care environments, Supported Independent Living arrangements, while also providing leadership and operational oversight to Support Workers delivering in-home and community-based NDIS services.

This is not just a desk-based leadership role.

About the Role

As an NDIS Service Team Leader, you will take a proactive, hands-on approach to leading daily service operations, supporting frontline teams and ensuring participants receive safe, consistent, responsive and high-quality person-centred care.

A key focus of this role is building strong relationships, maintaining proactive and effective communication, and working collaboratively with participants, families, Support Workers, Case Managers and other key stakeholders to ensure seamless service delivery and positive participant outcomes.

You will demonstrate strong leadership, initiative and problem-solving skills, proactively identifying and resolving operational challenges, responding to changing participant needs and supporting your team to deliver exceptional care.

As an NDIS Service Team Leader, you will play a key role in leading frontline teams, overseeing daily service operations and ensuring exceptional, person-centred support.

- Service Operations & Coordination: Oversee the day-to-day running of 24/7 care homes, SIL and independent living arrangements, as well as in-home and community-based NDIS services, ensuring safe,



consistent and high-quality support.

- Team Leadership & Development: Lead, mentor and support Disability Support Workers, promoting a positive team culture, accountability, effective communication and professional development.

- Complex Care & Behaviour Support: Guide teams in supporting participants with complex physical, psychosocial, mental health and behavioural needs, ensuring individual care plans, positive behaviour support strategies and risk management procedures are followed.

- Participant & Customer Relationships: Build strong relationships with participants, families and stakeholders, ensuring responsive, person-centred services that promote independence, choice and participant satisfaction.

- Hands-On Leadership: Maintain a visible presence across service locations, provide practical guidance to frontline teams and deliver direct participant support when required.

- Quality, Safety & Compliance: Monitor service standards, staff performance, documentation, incident management and compliance with NDIS requirements and organisational policies.

- Problem-Solving & Continuous Improvement: Proactively manage staffing and operational challenges, identify opportunities for service improvement and work collaboratively with management to achieve positive participant and team outcomes.

Qualifications & Requirements

Applicants will require:

- Relevant qualification in Disability, Community Services, Individual Support, Mental Health or a related discipline,



or equivalent relevant experience.

- Previous experience in a Team Leader in disability services or similar leadership position is essential.

- Strong understanding of SIL, 24/7 supported care, and community-based disability support services.

- Experience supporting participants with complex needs, including behaviours of concern and psychosocial disabilities.

- Sound knowledge of NDIS Practice Standards, safeguarding responsibilities and person-centred support.

- Willingness to travel between service locations across Adelaide's metro suburbs.
