The Community Health Worker is responsible for providing culturally appropriate healthcare services and information to community members with the goal of improving health outcomes. They work in a variety of settings, including clinics, hospitals, community centres, and outreach programs.
Responsibilities:
Conduct community assessments to identify healthcare needs and develop health education programs.
Organise health screenings and outreach events in the community to provide access to health resources and services.
Identify health disparities and work with local health organizations to improve access to care for disenfranchised groups.
Build trusting relationships with community members, providing education on healthy living, chronic disease management, preventive care, and disease self-management.
Advocate for individual and community healthcare needs to healthcare providers, policymakers and other relevant organisations.
Assist with coordination of referrals to medical specialists, social services, and other community resources.
Maintain accurate records of interventions, referrals and care coordination efforts.
Qualifications:
High school diploma or equivalent, with some formal training in public health, community organising or related field or a bachelor’s degree in public health, community health or a related field preferred.
Bilingual capability in English and other languages, depending on the community served, is a plus.
Ability to work independently and create effective workflows to manage tasks and responsibilities.
Experience interacting with diverse communities and building positive relationships.
Proficient with computer programs and electronic health record systems.
Understanding of HIPAA privacy rules and ability to maintain confidentiality.
