We deliver Supported Independent Living (SIL), In-Home Care, Community Participation, Short-Term Accommodation (STA), Community Nursing, and Allied Health Services.About the RoleWe are seeking an experienced and compassionate Registered Nurse – Care Coordinator to join our team on a casual basis (3 days per week, 8-hour shifts).
In this role, you will provide direct nursing support to participants while ensuring their health, safety, and well-being.
You will also play a key role in training and supporting staff to deliver high-quality, individualised care.Key ResponsibilitiesDeliver person-centered nursing care to NDIS participantsDevelop, implement, and review individualised care plansProvide clinical guidance, supervision, and training to support workersConduct health assessments and ongoing monitoringSupport behaviour management strategies, including restrictive practicesCollaborate with families, carers,
and multidisciplinary teamsMaintain accurate documentation and ensure NDIS complianceRespond to complex care needs and crisis situationsSkills & AttributesStrong client-focused approach with excellent communication skillsAbility to adapt communication style to suit different audiencesSound computer literacy (Microsoft Office, email systems)High level of confidentiality and professionalismKey CompetenciesMinimum 2 years 'experience in disability, mental health and complex care Minimum 2 years' experience in community nursing (preferred)Understanding of restrictive practices and behaviour supportExperience supporting individuals with challenging behavioursExperience in Supported Independent Living (SIL)Experience supporting people living with dementiaExperience in training and developing staffQualifications & RequirementsCurrent AHPRA registration as a Registered NurseExperience in disability, community nursing, or allied health settingsAbility to lead care teams and manage conflictStrong skills in care planning, report writing, and complianceAbility to work autonomously and within a multidisciplinary teamExcellent time management and organisational skillsCurrent Driver's LicenceNDIS Worker Screening ClearanceWorking with Children Check (Blue Card)What We OfferFlexible casual work arrangement (3 days per week)Supportive team environmentOngoing skilled developmentOpportunity to make a meaningful impactHow to ApplyApply now with your resume outlining your experience and suitability.
