Key Worker Responsibilities: n
n
Building relationships with the support team,familyand social network
n
Coordinating and overseeing flexible and responsive provision of supportsacross several providers
n
Managing weekly schedules, daily activities, personal
administration,budgetingand
travel
n
Clearly communicating plans,schedulesand expectations to the support team
n
Coordinating therapy and medical appointments and supporting outcomes
n
Completing
inductions,trainingandbuddyshifts
with new workers
n
Effectively communicating changes to rostered hours
n
Monitoring and responding to all team communicationsin a timely manner
n
Incident management, support and debriefing in collaboration with thefamilyreceiving support
n
Key Worker Profile: n

A commitment to long-term relationships with individuals seeking support and their families
n
Demonstrated integrity, initiative, and alignment withfamily governedprinciples and support models
n
Understanding and experience of individuals whodirecttheir own supports
n
Experienceinpractical
everydayknowledgerequiredto
run a sustainable and healthy household
n
Solid connections within the local community or a demonstrated willingness to build relationships alongside individuals with disabilities
n
High-level interpersonal communication skills, with an ability to connect with people of all communication abilities
n
Ability to work both independently as well as to inspire, motivate,coachand supervise a team of support workers
n
Shifts / Hours: n
n
Some direct Support Shiftswith the family,7-10am & 7pm-10pm
n
Variable / flexible shifts throughout the week for indirect support / Key Worker duties
n
22 hours per week available to start
n
Starting Pay Rates: n
n
Monday to Friday 6am to 8pm shift - $***** per hour
n
Monday to Friday 8pm - 6am shift - $***** per hour
n
Saturday - $***** per hour
n
Sunday - $***** per hour
n
Public Holidays - $***** per hour
n
