Lifestyle Solutions is seeking a compassionate Disability Support Worker in Albion Park Rail, NSW. You will assist customers to become more independent at home and in the community, supporting personal care and daily living tasks.

Responsibilities include mentoring, coaching independent living skills, administering medication per plans, documenting handovers and incident reports, and helping customers access community programs. A current driver's license and NDIS clearance are required.
