Key Responsibilitiesn
n
Deliver high-quality, person-centred care in clients' homes and in our SIL
n
Support clients with daily living activities including personal care, mobility, and meal preparation
n
Provide compassionate care for individuals living with dementia and complex needs
n
Promote independence, dignity, and wellbeing at all times
n
Observe, document,



and report changes in client condition to the nursing team
n
Follow care plans and work collaboratively within a nurse-led model
n
Build trusting relationships with clients and their families
n
Maintain skilled standards, confidentiality, and compliance with policies and procedures
n
Desired Qualifications & Skillsn
n
Certificate III or IV in Aged Care, Disability, or Individual Support (or currently studying nursing)
n
First Aid and CPR
n
Current Manual Handling (or we can provide)
n
NDIS modules mandatory
n
Current police check
n
WWVP
n
Must have workers screening ID
n
Medication training for cert 4
n
Strong understanding of person-centred care principles
n
Experience in home care and/or dementia support (preferred)
n
Excellent communication and interpersonal skills
n
Ability to think critically and respond to changing client needs
n
Reliable, organised, and able to work independently
n
Commitment to high standards of care and professionalism
n
Current driver's licence and willingness to travel between clients
n
