About the Role
This is a highly autonomous community nursing role supporting clients across Melbourne's north and western suburbs.
You'll deliver nursing assessments, develop care plans, monitor client wellbeing, collaborate with healthcare professionals and provide leadership and clinical support to our care workforce.
The role includes professional nursing care, clinical oversight, staff support and ensuring care aligns with relevant professional standards and legislative requirements.
A key part of your success will be supporting and enabling our growing field workforce.
You'll provide clinical guidance, coaching and education to care teams, helping them deliver safe and high-quality care while building capability across the organisation.
The role includes leadership, supervision and clinical support of staff and providing guidance to client care teams.
What We're Looking For
We're seeking someone who has:
Current AHPRA registration as a Registered Nurse.
Proven experience working within
community aged care
, community nursing, home care or related settings.
Prior experience in geriatric care, community nursing and aged care is required.
Strong clinical assessment and care planning skills.
Confidence making independent clinical decisions and managing competing priorities.
Experience educating, mentoring or supporting care workers and multidisciplinary teams.
Excellent communication and relationship-building skills.
A genuine passion for helping older Australians remain independent in their own homes.
The Reality of the Role
Our clients are spread across a
significant geographic area throughout Melbourne's north and western suburbs
, so you'll need to be comfortable spending time on the road and travelling between client homes.
You'll enjoy variety every day, but success in this role requires someone who is organised, adaptable and genuinely enjoys working within a community-based model of care rather than a single-site workplace.
