About the role
Working within a multi-disciplinary team environment (telehealth / face to face) this position focuses on the provision of government funded carer support services that are responsive to the needs of the local community.
This position encompasses a variety of roles including information provision; guided referrals; facilitation of peer support groups; community outreach; developing carer capacity and resilience; delivering practical and innovative practical support.
Key Responsibilities
Undertake holistic assessments of clients' need and develop goal-directed care plans
Respond to emergency, practical and emotional needs of carers
Coordinate service delivery and refer to other service types as required
Contribute skills, knowledge and expertise to promote community awareness and understanding of carer issues though outreach and promotional activities
Providing carer coaching and support to carers to reach their goals
What we are looking for
Relevant qualification in Social Work, Welfare, Aged Care, Mental Health, Disability or Community Case Management, with an understanding of the needs of carers and the people they support.
Excellent communication and interpersonal skills,



with the ability to build effective relationships and work collaboratively as part of a team or independently.
Robust organisational and digital skills, including the ability to manage competing priorities and use Microsoft Office, databases and content management systems effectively.
Commitment to continuous learning and quality care, with public health sector experience and consumer group facilitation skills highly regarded.
