Our mission is to improve the quality of life and wellbeing of every person we support.About the RoleWe are currently seeking a reliable, and capable Care Coordinator to join our team in Sydney Central and Eastern Suburbs.The Care Coordinator is responsible for the effective coordination and management of client services under the supervision of Care Manager.
This includes client assessments, care planning, service coordination, and case management.The successful candidate will work closely with clients, families, support workers, health professionals, and internal management to ensure services are appropriately planned, coordinated, delivered, and reviewed.
Dependable performer has the opportunity to transition into a full time role and advance into a management position.Key Responsibilities:Client Assessment and Care PlanningConduct assessments and reviews for clientsAssess clients' individual needs, preferences, goals, risks, and support requirements.Develop, implement, and regularly review person-centred care plans.Monitor changes in clients' health, wellbeing, circumstances,
and support needs.Coordinate appropriate services and make recommendations based on client requirements.Case Management and Service CoordinationProvide ongoing case management and coordinate services to ensure quality and continuity of care.Liaise with clients, families, support workers, health professionals, and other stakeholders.Coordinate referrals and collaborate with external service providers where required.Monitor service delivery and address issues or changes in client needs promptly.Respond appropriately to incidents and emergency situations and escalated concerns in accordance with organisational procedures.Selection CriteriaDiploma of Nursing, Community Services or equivalent preferredHaving experience of working as Personal Care Workers, Aged Care Workers, or Disability Support Workers in community or home care, orExperience in case management, service coordination, care coordination, program development, or client assessment.Skills and Personal AttributesGood verbal and written communication skills.Oganisational, coordination, and time-management skills.Ability to manage multiple clients, services, priorities, and deadlines.Attention to detail and accurate documentation skills.Ability to work independently and as part of a multidisciplinary team.Problem-solving and decision-making skills.Willingness to learn, develop professionally, and take on new challenges.Understanding of person-centred care and client rights.Mandatory Compliance RequirementsCurrent National Police Check.Current First Aid and CPR certificates.Current unrestricted Australian driver's licence, car insurance, and vehicle registration.Current Working With Children Check and NDIS Worker Check.
