You will provide psychosocial support, advocacy, and case management to residents and families, especially during hospital-to-care transitions.Requirements: Bachelor of Social Work (or equivalent) and eligibility for AASW, with experience in health or aged care.
You will work with a multidisciplinary team to assess needs, coordinate referrals, and advocate for residents'
