Wide Bay Hospital & Health Service is seeking a Senior Social Worker to provide high level clinical services within the Integrated Care Rehabilitation Service on the Bundaberg/ Fraser Coast site.
The role supports the Transition Care Program, focusing on rehabilitation for older patients after acute care and helping prevent unnecessary readmissions.You will supervise Allied Health Assistants and contribute to a cooperative, patient-centred care approach.
