IRT Group is hiring a Home Care Partner to work with customers in NSW, ACT and Qld, helping them stay at home while meeting care needs.You will manage cases, coordinate services, and review plans to support wellbeing.
This role requires case management qualifications and 1–2 years in aged care, with hybrid work from office and home.
Eligible regions include ACT, Far South Coast, Shoalhaven, Illawarra or South East Sydney/ South West Sydney.
