Pearl Home Care is seeking a compassionate and dedicated Registered Nurse to join our team in Geelong and Ballarat, VIC.
This is a part-time position that offers the flexibility to balance your qualified commitments with your personal life.
As an Enrolled Nurse in our aged care services, you will play a vital role in delivering high-quality, person-centered care to our clients in their homes and community settings.
This position is integral to our mission of providing exceptional aged care support that enhances the quality of life for our clients whilst maintaining their independence and dignity.
Pearl Home Care is seeking an experienced and motivated Registered Nurse to support clients under the Support at Home (SAH) Program.
The role is based onsite at our Geelong office, with travel throughout Geelong and Ballarat for home visits and clinical assessments.
Complete comprehensive initial and ongoing nursing assessments.
Complete home visits and clinical reviews.
Develop, review and update individualized care plans.
Complete Personal Care and ADL assessments where required.
Complete falls risk assessments, post-fall reviews and appropriate interventions.
Identify and respond to deterioration or changes in a client's health or functional status.
Complete clinical risk assessments and risk-management plans.
Escalate clinical concerns promptly and implement appropriate follow-up actions.
Complete accurate progress notes, nursing notes and clinical documentation.
Maintain complete and up-to-date client clinical records.
Review client documentation and ensure clinical information remains current.
Complete incident assessments, investigations, follow-up and documentation.
Assess incidents for potential SIRS reporting requirements and assist with SIRS management and reporting.
Develop and document corrective actions and preventative strategies following incidents.
Liaise closely with Case Managers/Care Partners regarding changes in client needs, risks and clinical recommendations.
Liaise with GPs, hospitals, allied health professionals, families and other health professionals.
Make appropriate clinical referrals and recommendations.
Participate in case conferences and multidisciplinary care planning where required.
Provide clinical guidance and support to support workers and other staff.
Support safe delivery of personal care and other clinically related services.
Monitor clients with complex or changing clinical needs and arrange appropriate follow-up.
Ensure clinical practice and documentation meet Support at Home, Aged Care Quality Standards and organisational requirements.

What We Are Looking For
Current AHPRA registration as a Registered Nurse – essential.
Current Australian driver's licence and reliable vehicle.
Excellent time management and organisational skills.
Strong critical thinking and clinical judgement.
Able to assess a situation, make appropriate clinical decisions and settle where required.
Able to work independently and manage a caseload.
Honest, reliable and committed.
Quick learner who can adapt to new systems and processes.
Confident working with Case Managers, GPs, allied health professionals and families.
Previous aged care, community nursing or Support at Home/Home Care experience is highly regarded.
Strong knowledge of aged care practices, standards, and regulatory requirements
Excellent interpersonal and communication skills with the ability to build rapport with clients and families
Proficiency in clinical assessment, wound care, and medication administration
Ability to work autonomously and make sound clinical judgements within your scope of practice
Physical fitness and ability to provide hands‐on care and support mobility assistance
Commitment to maintaining confidentiality and upholding professional ethics
Current First Aid Certificate or willingness to obtain one
Immunization records current as per aged care requirements
