You'll provide transitional support and care for people living with very severe behavioural and psychological symptoms of Dementia, ensuring their safety, dignity, and wellbeing at every stage of care.KeyresponsibilitiesDelivery of care and services as outlined in the resident care plan that is consistent with relevant standards and legislation and within scope of practiceMake adjustments to care delivery and care plan to reflect residents current care requirements.Implement system changes that improve care and service delivery for all residentsWhat we're looking forCertificate III or Certificate IV in Aged CareSound literacy and documentation skillsValid NDIS Worker Screening ClearanceExperience in aged care or inpatient care,
