The role is a part time ongoing position (0.5 EFT) within the hospital and community care setting, focusing on preventing avoidable hospital presentations and coordinating care after discharge.You will assess needs, coordinate services, manage a caseload, and develop personalised plans for patients with chronic conditions, working with an interdisciplinary team to promote
