Top Care Community Services Pty Ltd (TCCS) is a registered National Disability Insurance Scheme (NDIS) service provider based in the Northern Territory, primarily providing Support Coordination services. We support our participants to develop sustainable capacity and lead a healthy, content life with enhanced social and family relationships.
We are seeking a compassionate and well-organised person who is experienced in NDIS processes to join our team.
The successful candidate will work closely, yet autonomously, with the team and participants to deliver a high level of Specialist Support Coordination.
As a Specialist Support Coordinator / Team Leader, the successful candidate will hold a direct caseload of complex disability needs participants while leading the practice quality of our Support Coordination team.
Location: Darwin, Katherine and Alice Springs
Key responsibilities:
· Support participants with complex psychosocial, behavioural, medical and / or forensic needs and advocate for participants navigating complex service systems.
· Support participants to understand their NDIS plan, goals and objectives while promoting choice and control and supported decision making.
· Identify barriers and put practical strategies in place.
· Identify, connect, and coordinate appropriate service providers and community resources that meet the participant's needs in accordance with their allocated NDIS funds.
· Monitor NDIS budgets and service agreements.
· Work closely with various stakeholders including service providers, allied health professionals, and community organizations to ensure seamless service delivery.
· Provide assistance during times of crisis, ensuring continuity of support and appropriate intervention where necessary.
· Maintain accurate records of participant progress, develop reports, and communicate regularly with the NDIS regarding plan outcomes and reviews.
· Ensure compliance with all relevant legislation, NDIS guidelines and maintain up-to-date knowledge of any changes to the NDIS framework.
· Complete clear, outcome-focused reports for the NDIA.
· Provide Supervision, mentoring and induction to Support Coordinators.
· Support strategic planning, program growth and service innovation.
Selection Criteria:
· Bachelor qualification in Psychology, Social Work, OT, Mental Health, Community Services or similar with current registration with the relevant regulatory agency.
· Strong cultural awareness and experience supporting multi-cultural, Aboriginal and Torres and Strait Islander participants.
· A minimum three (3) years' experience as a NDIS Support Coordinator or NDIS LAC.
· Demonstrated a solid knowledge and understanding of the NDIS including mandatory reporting, Quality and Safeguards, NDIS Price Guide and relevant legislations.
· Confident working with participants with complex and high-risk needs.
· Experience within community connecting and building relationships with service providers, individuals and/or families/carers.
· Must be able to relay information to participants so they understand.
· Strong written, administration and computer skills for the creation of case notes, reports, correspondence and other documents in a cloud-based management system.
· Strong interpersonal skills with a positive attitude and a versatile and adaptive nature to respond to the changing needs of our participants.
· Excellent organizational and time management skills.
· Able to work independently while being part of a supportive team.
· Applicants will be subject to a NDIS Worker Screening Check, Working with Children Check, and four (4) NDIS mandatory eLearning modules.
· National Police Check
· Current First Aid and CPR
· COVID booster Vaccination
· Australian Drivers Licence, and a reliable vehicle with registration and insurance
· Right to work in Australia.
