Part-time position flexible hours – 35 hours a fortnight

The Care Finder Officer will provide culturally responsive, person‑centred support to individuals who may experience barriers to accessing services, including language and literacy difficulties, social isolation, digital exclusion, homelessness, cognitive decline, mental health concerns or limited informal support.

Key responsibilities include:

- Providing intensive one-to-one service navigation and assertive outreach.
- Supporting clients through My Aged Care referrals, assessments and follow‑up.
- Building trusting relationships with clients, carers and families.
- Identifying and addressing barriers to accessing aged care and community services.
- supporting clients and coordinating warm referrals to appropriate health, housing and community supports.
- Maintaining regular client contact and accurate, timely case notes.
- Developing effective relationships with community organisations, service providers and other stakeholders.




- Collecting client feedback and program data to support reporting and continuous improvement.

Essential Experience & Qualifications

- Tertiary qualifications (minimum Certificate 4) in Social Work, Community Services or other discipline relevant to the role
- An understanding of the Australian aged care system and issues affecting older people and their carers.
- Experience working with CALD communities and an understanding of the needs of older people from diverse cultural backgrounds.
- Solid communication, relationship‑building and advocacy skills.
- The ability to work independently, manage competing priorities and contribute effectively within a team.
