Latrobe Regional Health is seeking a HIP Care Coordinator to join the Health Independence Program team in Gippsland. The role is a part time ongoing position (0.5 EFT) within the hospital and community care setting, focusing on preventing avoidable hospital presentations and coordinating care after discharge.

You will assess needs, coordinate services, manage a caseload, and develop personalised plans for patients with chronic conditions, working with an interdisciplinary team to promote
