Peninsula Health in Frankston is seeking an experienced Older Adult Community Mental Health Clinician to join the specialised multi-disciplinary Older Adult CMHT. You will plan, evaluate and review care, undertake face-to-face assessments and contribute to recovery care plans for older people with severe mental health disorders.

Role involves frequent liaison with external services and internal teams, providing care coordination and supporting carers.
