About the opportunity:
This pivotal leadership role is responsible for deliveringhigh-quality, person-centred home care services that empowers olderpeople to live with dignity, safety and independence in their ownhomes. Leading the day-to-day operations of a community aged carehub, the role oversees the planning and coordination ofindividualised care, ensuring services align with each client’sgoals, preferences and needs. At the same time, it provides strongleadership and support to a multidisciplinary team, fostering apositive and high-performing culture focused on safe, high-qualityperson-centred care outcomes.
Blending operational oversight with a genuine commitment topeople, the role brings together client advocacy, team leadership,financial management and stakeholder engagement. Working closelywith clients, families and health professionals, it ensures everyinteraction is respectful, responsive and outcomes focused. With astrong focus on continuous improvement, service quality andsustainable performance, this role plays an important part inshaping the future of community aged care within the region.
Your day-to-day will involve:
- Overseeing day-to-day operations, including rostering,service delivery, care planning and documentationstandards
- Managing and mentoring a team of clinical staff, carepartners, carers and support staff to deliver exceptionalcare
- Ensuring compliance with the Aged Care Act, QualityStandards and funding program requirements
- Monitoring budgets, utilisation and financial performance,identifying and addressing variances
- Building strong relationships with clients, families,health professionals and community stakeholders
- Driving continuous improvement,
implementing change andembedding best practice across the service
About you:
You are a confident, compassionate and resilient leader whothrives in a fast paced, people focused environment. You bring astrong understanding of community aged care, balanced withcommercial awareness and the ability to lead teams through sectorchange. You bring proven leadership skills with experience insupervising and developing high performing teams. Above all, youare values driven, with a genuine commitment to delivering safe,high quality care and improving outcomes for older people to remainat home in the community.
- Qualifications in Aged Care, Nursing, Allied Health orCommunity Services
- NDIS Worker Screening Clearance, Fiduciary check, Driver’sLicence
- Strong working knowledge of aged care legislation, QualityStandards and funding programs
- Proven experience leading community or home care servicesand multidisciplinary teams
- Demonstrated financial and operational managementcapability
- Ability to drive change, continuous improvement and servicegrowth
