This is a temporary full-time role (38 hpw) up to 30 June 2028 with a possibility of extension.
Campbelltown Medicare Mental Health Centre provides a welcoming entry point for people aged 16 years and older and their support people experience distress or mental health challenges to access support services. Our multidisciplinary team brings clinical, peer, cultural and community expertise together to provide coordinated support that reflects people's lives, relationships and wellbeing,
The role works collaboratively with consumers to connect with services and supports in the community, facilitating person-centred and integrated care. You will deliver practical support through information sharing and education, identifying and addressing barriers to access and facilitating warm referrals, ensuring consumers are linked to the right service at the right time.

Skills & Experience
- A tertiary qualification in a relevant discipline, and/or extensive experience in mental health, health,
community services or social sciences.
- Demonstrated understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems.
- Demonstrated understanding of supports and services available in the local and regional communities.
- Demonstrated ability to engage with consumers and their families or carers to provide care navigation and referral support.
