St Vincent's Health Australia seeks an experienced Social Worker to join the ALERT team as an Emergency Department Care Coordinator at St Vincent's Hospital Melbourne. You will identify and assess patients with complex health and social needs and coordinate care across hospital and community services to support safe discharge.

Working in an interdisciplinary ED workplace, you will deliver outreach case management after discharge, collaborate with families and healthcare professionals, and help
