The House Lead is responsible for the day-to-day operational leadership of the client's home. The role ensures safe, compliant and person-centred service delivery while supporting staff performance, team stability and service continuity during organisational growth. You will provide direct support to the client while leading and supporting the wider care team. This includes clinical monitoring, high intensity support, staff supervision, competency assessment, care planning, documentation, incident management and roster coordination.

Key responsibilities

- Provide direct, hands-on support to the client 1:1 in their home and community
- Deliver high intensity clinical supports, including suprapubic catheter changes (RN Registered only task), continence management, health monitoring and clinical observations
- Support the client with personal care and activities of daily living
- Provide safe manual handling, positioning and transfer support for a full-time power wheelchair user
- Monitor the client's health status and clinical presentation and identify and elevate changes or concerns promptly
- Provide day-to-day operational leadership within the client's home and help ensure the home is protected, organised, compliant and operating consistently
- Provide practical guidance to support workers during shifts and support staff performance, confidence and stability
- Work collaboratively with the other House Lead to provide consistent leadership and maintain clear communication between shifts
- Provide staff supervision and support to workers delivering disability health supports including high intensity supports and conduct inductions, buddy shifts and oversee competency assessments or conduct competency assessments if RN.
- Develop,



review and update or consult for the clinical care plans and health support documentation and maintain accurate, timely and audit-ready documentation

About you

- Current AHPRA registration as a Registered Nurse (preferred)
- Endorsed Enrolled Nurse (desirable), with significant experience in nursing, leadership and staff management.
- Certificate or Diploma in a relevant disability or aged care or nursing field (desirable) with significant experience in disability care, catheter care, manual handling, IT, staff leadership.
- Demonstrated experience delivering and leading high intensity clinical care
- Experience with catheter care, suprapubic catheter management, complex bowel care & Dysphagia
- Experience supporting person with significant physical disability and complex health needs
- Experience working with clients who use a power wheelchair full-time
- Experience training, assessing or supervising support workers
- Sound understanding of the NDIS High Intensity Daily Personal Activities framework
- Robust clinical assessment, escalation and risk management skills
- Current First Aid and CPR certification
- Current NDIS Worker Screening Check clearance & NDIS Quality, Safety & You Certificate
- An experienced superstar with IT and computer systems, Quality Management, Rostering and report writing.

Yellowbird Enterprises is seeking an experienced and compassionate Registered Nurse to join our team. The organisation is committed to secure, compliant and person-centred service delivery.

You will be part of 24/7 roster of care. Likely monday-friday roster, rotating weekends, and available to backfill and roster self for or as needed. You will be rostered over the xmas new year period. 3-month initial casual contract with guaranteed hours, to establish right fit for the client and Yellowbird to lead this team jointly. 64-76hrs per fortnight, with view to transition to ppt or pft.
