What you will do





- Manage a complex Adult Social Care caseload in an effective and person-centred manner.
- Undertake comprehensive Care Act assessments, reviews, and support planning.
- Complete Mental Capacity Assessments and facilitate Best Interests decision-making processes.
- Lead safeguarding enquiries, assess risks, and implement appropriate protective interventions.
- Prepare detailed reports, assessments, and Court of Protection documentation where required.
- Work collaboratively with health professionals, care providers, and multi-agency partners to coordinate effective support and interventions.
- Monitor, review, and evaluate care packages to ensure desired outcomes are achieved and resources are used effectively.
- Maintain accurate, timely, and legally compliant case records in line with legislation, policies, and professional standards.
- Participate in duty functions and contribute to the ongoing development and improvement of the Adult Social Care service.
- Promote independence, wellbeing, and strengths-based practice in all aspects of your work.
