BaptistCare is seeking a Care Facilitator to support older people in Goulburn and surrounding areas, enabling independent living at home. You will plan individualized care and collaborate with families to set achievable goals.

This role requires empathetic communication, a valid driver’s license, and comfort working in community settings with a multidisciplinary team. A hybrid work arrangement is offered.
