Job Description

Purpose of Role

The Personal Care Assistant provides high-quality, person-centred care and support to Home Care clients in their own homes.

The role demonstrates a commitment to best practice in aged care by delivering services that promote each client’s dignity, independence, comfort, safety and quality of life . Care and services are provided in accordance with individual care plans, organisational policies and relevant aged care standards.

The Personal Care Assistant is responsible for monitoring clients’ wellbeing and promptly reporting any changes in a client’s health, condition or care needs to the Registered Nurse, Care Partner or other appropriate member of the care team .

Key Responsibilities

- Deliver person-centred care and support that respects each client’s individual needs, preferences, choices, culture and circumstances
- Use appropriate communication strategies to meet the diverse and complex needs of clients while building positive relationships and maintaining skilled boundaries
- Provide agreed care and assistance within the scope of the role and relevant qualifications, including Certificate III in Individual Support (Ageing) or equivalent .
- Develop appropriate professional relationships with clients and support the identification of their strengths, needs, abilities, preferences and available resources.
- Deliver care and services in accordance with the client’s documented care plan , organisational policies, procedures and applicable aged care standards.




- Assist clients with activities of daily living as outlined in their care plan, which may include:
- showering, bathing and personal hygiene
- grooming and skin care
- toileting and continence support
- mobility, transfers and positioning
- assistance with eating and drinking
- medication support within the employee’s authorised scope
- therapy or exercise support as directed
- social engagement and companionship
- meal preparation
- domestic assistance and other approved home care services.

- Observe clients and promptly report changes in health, behaviour, wellbeing, mobility or care needs to the appropriate member of the care team.
- Promote client choice, independence and participation , encouraging clients to maintain their abilities wherever possible.
- Use appropriate care strategies to promote clients’ self-esteem, dignity, privacy, safety, security and comfort .
- Prioritise workload effectively and respond appropriately to changing client needs and circumstances.
- Use equipment, supplies and other healthcare resources safely, appropriately and efficiently.
- Identify and escalating risks, incidents, hazards or concerns in accordance with organisational policies and procedures.
- Work collaboratively with Registered Nurses, Care Partners and other members of the multidisciplinary care team to support continuity and quality of care.
- Participate in continuous improvement activities and contribute feedback about the effectiveness of care practices, policies and procedures.
- Maintain accurate, timely and confidential client documentation and records in accordance with organisational requirements.

Desired Skills and Experience

What you will need to be successful:





- Certificate III Individual Support – Ageing or equivalent
- A current Drivers License and a car with comprehensive car insurance
- Current National Police Check or willingness to obtain
- Demonstrated ability to work to required resident plans.
- An ability to work as an individual or within a team environment.
- Effective written and verbal communication skills as well as the ability to document resident information accurately.
- Ability to use workplace technology including accessing the intranet and work emails, apps, complete on-line training, submit leave requests and other employee functions.
- Previous experience working in residential Aged Care
- Knowledge of the Strengthened Aged Care Quality Standards and person-centred care principles
- A current First Aid and CPR certificate
