The Opportunity
We are seeking an experienced Care Partner with hands-on Support at Home experience to join our team.
This is an exciting opportunity to use your knowledge and experience to support clients in achieving the outcomes that are most important to them.
You'll develop genuine face-to-face relationships with clients and their families, understand their individual goals and circumstances, and turn those conversations into practical, personalised support.
You'll be supported by experienced leaders, clear processes and modern digital systems designed to streamline administration and give you more time to focus on your clients.
Working across a defined caseload, you'll play an important role in coordinating high-quality care, identifying opportunities for improvement and ensuring clients receive responsive and effective support.
This role will suit someone who enjoys client conversations, embraces technology and change and is motivated to help shape the future of aged care.
Key Responsibilities
- Build strong, trusting relationships with clients and their families through regular and meaningful engagement
- Understand client goals and translate them into personalised, compliant care plans
- Regularly review care plans and proactively adjust supports as client needs change
- Empower clients to make informed decisions about their services, funding and priorities
- Monitor and manage funding to support positive outcomes, sustainability and value
- Identify emerging risks, service gaps and wellbeing concerns and take appropriate action
- Coordinate services, supports and referrals to ensure care is seamless and responsive
- Manage incidents, feedback and complaints with empathy, professionalism and accountability
- Maintain accurate, high-quality and audit-ready client documentation
- Work collaboratively with care workers, clinicians and internal teams to achieve consistent client outcomes
- Utilise digital systems and technology effectively to remain organised and maximise time spent with clients
About You
You're someone who genuinely cares about people, while also bringing confidence, sound judgement and the ability to navigate complex situations.
You enjoy connecting with people,
having meaningful conversations and helping clients make informed decisions about their care. You're proactive, resilient and calm under pressure, with the ability to balance empathy and advocacy with accountability and compliance.
You're also comfortable working in an environment where change and new technology is part of the journey. You see opportunities to learn, improve and contribute rather than simply maintaining the status quo.
Most importantly, you bring both compassion and capability - a genuine desire to make a difference combined with the professionalism needed to deliver outstanding outcomes.
You'll also bring:
- Experience within aged care, community services, health or a related sector
- A relevant qualification in social work, allied health, community services or nursing
- Understanding of Home Care Packages and/or the Support at Home program
- Confidence managing a caseload and taking ownership of client outcomes
- Strong communication and relationship-building skills
- The ability to balance client advocacy with compliance and funding requirements
- Confidence using digital systems, CRM platforms and care management technology
- Adaptability, resilience and a willingness to embrace new ways of working
