Pay Rate: $2000 Weekly Gross pay (Non-taxabletravel/housing and M&I; stipend available for qualified/eligible travelers)
MileageReimbursement: $0.49/mile for distancecovered between patient home appointments
Unit: Home Health
Shift: 8 Hours; Days, 8a – 5p
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri; Every 4th Weekend, and On-call once weekly as needed
Position Requirements:
- Registered Nurse: 2years - Required
- Recent/Current HomeHealth Nursing experience: 1 year within the last 3 years – Required
- OASIS experience: 1 year– Required
- OASIS admissions and Case Management - Required
- Home Care Home Base(HCHB) charting experience – Preferred
- Timely communication and documentation - Required
- Must have validDriver’s License and Personal Auto Insurance
- Must be Covid Vaccinatedor provide medical or religious exemption
Education, Licenseand Certification Requirements:
- ASN or an equivalent– Required
- BSN or an equivalent– Highly preferred
- RN License for Stateof Ohio or Compact - Required
- BLS Certificationfrom AHA or American Red Cross - Required
Job Description:
- Provide andcoordinate care for assigned patients in comfort of their homes.
- Develop and implementnursing care plans including Start of Care and admissions as assigned.
- Support patientrecovery and ability to stay at home - enabling them to spend quality timewith their loved ones and doing the activities they enjoy.
- Empower patients andfamilies to reach healthcare goals by educating them on disease management.
- Provide Home health nursingcare and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
- Ensure patients’status is assessed on an ongoing basis and pertinent information isgathered and documented.
- Notify physicians ofsignificant changes in status.
- Must communicate with the office in time manner,
- Must document and completecharting for all patient visits in a timely manner in accordance with client policies and procedures.
