This is a short-term contract role for an experienced Clinical Pharmacist at a regional public hospital in VIC. The role is suited to pharmacists with hospital experience who are confident working independently across wards in a supportive team workplace. Dispensary pharmacists keen to transition into a clinical hospital pharmacy role are encouraged to apply, with comprehensive training and support provided.
Key responsibilities
Deliver clinical pharmacy services across general medicine, surgical, and rehabilitation wards.
Conduct medication reconciliations, clinical reviews, and discharge counselling to support safe and effective patient care.
Assist with patient admissions, discharge planning, and continuity of care.
Participate in multidisciplinary ward rounds, working closely with medical, nursing, and allied health teams.
Contribute to daily pharmacy operations and quality patient care initiatives.
About You
Current and unrestricted AHPRA registration as a pharmacist.
Previous experience working in an Australian hospital pharmacy setting, whether public or private.
Ability to work autonomously in ward-based clinical environments.
Excellent communication skills with a collaborative and patient-centred approach.
