Community Access Care is seeking a compassionate, experienced and motivated Registered Nurse to join our team. This role is ideal for a nurse who is passionate about delivering high-quality, person-centred care and supporting people with disability, including participants with high and complex needs.

About the Role

As a Registered Nurse with CAC, you will be responsible for delivering and coordinating quality nursing care across participants' homes and community settings. You will work closely with participants, families, support workers and other health professionals to achieve positive outcomes and maximise independence.

Key responsibilities include:

- Providing person-centred nursing care in accordance with professional standards, evidence-based practice, relevant legislation and the NDIS Code of Conduct .
- Assessing, planning, implementing and evaluating participant care, including goal plans, clinical interventions, medications and treatment.
- Monitoring and supporting participants with complex needs, challenging behaviours and mental health concerns .
- Reviewing and supporting the reduction of restrictive practices in accordance with NDIS requirements.
- Providing guidance, education and support to Disability Support Workers .
- Developing and delivering training and education to support workers as required.
- Working collaboratively with multidisciplinary teams, families and external service providers to coordinate participant care.
- Maintaining accurate and timely clinical documentation, assessments,



reports and data in accordance with CAC and NDIS requirements.
- Responding appropriately to clinical emergencies and maintaining accountability for your skilled practice.
- Building positive relationships with participants through face-to-face and telephone contact while providing a high level of customer service.
- Supporting interdisciplinary programs that promote participant independence and community participation.

About You

We are looking for a nurse who is passionate about disability, committed to person-centred practice and confident supporting others to deliver exceptional care .

You will ideally have:

- Current nursing registration and relevant qualifications.
- Demonstrated experience working with people with disability and/or complex support needs.
- Strong knowledge and practical understanding of the NDIS and NDIS practice requirements .
- Experience supporting people with complex behaviours, mental health concerns and high support needs.
- Knowledge and experience relating to restrictive practices and positive behaviour support.
- Strong clinical assessment, care planning and documentation skills.
- The ability to provide clinical leadership, mentoring, education and guidance to support workers and less experienced nursing staff.
- Excellent communication, interpersonal and stakeholder management skills.
- A genuine commitment to promoting participant choice, dignity, independence, rights and quality of life.
