Key responsibilities
- The role includes medication reconciliation, clinical review, drug information provision and independent therapeutic decision-making to optimise patient outcomes.
- Working as part of a multidisciplinary team, the position also contributes to service planning, quality improvement and regional outreach services.
- This role suits pharmacists with established hospital experience seeking greater clinical responsibility and autonomy.
- The role includes inpatient virtual clinical pharmacy and pharmacist-led outpatient clinics in pre-admissions, day infusions, renal and cardiology.
