Are you an experienced NDIS professional who is passionate about delivering high-quality, person-centred supports for people with complex needs?
Gladstone Community Linking Agency (GCLA) is seeking an experienced and motivated Senior NDIS Care Coordinator to join our NDIS team in Gladstone.
This is a key coordination role supporting participants with complex needs, high intensity supports and Supported Independent Living Services (SILS) . You will work closely with participants, families, Senior Support Workers, frontline teams, allied health professionals and other stakeholders to ensure supports are safe, responsive and aligned with each participant's goals.

About the Role
As a Senior NDIS Care Coordinator , you will coordinate and oversee the delivery of supports for participants with complex needs, ensuring services are tailored, safe, effective and responsive to individual goals, health needs and NDIS plan outcomes.
Your responsibilities will include:
- Coordinating the intake and onboarding of participants with complex support needs or SILS funding
- Developing, implementing and reviewing individual support plans in collaboration with participants, families and relevant professionals
- Coordinating supports for participants with high-intensity and complex care requirements
- Working closely with clinical practitioners including nurses, allied health professionals and medical teams
- Supporting the implementation and monitoring of Positive Behaviour Support Plans
- Ensuring restrictive practices are appropriately authorised, documented,
monitored and reported
- Monitoring participant risks, incidents and changes in support needs
- Coordinating internal and external services to ensure supports align with participant goals and NDIS funding
- Supporting participant transitions, including hospital discharge, changes in service arrangements and transitions between providers
- Maintaining current Schedules of Support and Rosters of Care
- Working closely with Senior Support Workers and frontline employees to ensure participant care plans, routines and support strategies are understood and followed
- Providing guidance, coaching and support to employees delivering complex care
- Building strong relationships with participants, families, guardians, allied health professionals and other stakeholders
- Maintaining accurate case notes, progress records, incident documentation and participant records
- Supporting NDIS compliance, audits, provider reporting and continuous improvement activities
- Identifying opportunities to improve participant outcomes, service quality and sustainability
The role requires strong communication, sound clinical and behavioural understanding and the ability to coordinate multiple stakeholders while maintaining a strong focus on participant choice, dignity, independence and safety .
What We’re Looking For
We are looking for an experienced NDIS professional who is confident coordinating complex supports and understands the importance of balancing participant choice, safety, quality, compliance and available funding.
You will be organised, solutions-focused and comfortable managing multiple priorities in a fast-paced environment. You will also have the confidence to provide guidance to frontline employees and work collaboratively with families, guardians, clinical teams and other service providers.
Diploma or higher qualification in Disability, Community Services, Allied Health Assistance, Mental Health or another relevant field
Demonstrated experience working within the NDIS and supporting people with complex needs
Knowledge and understanding of Supported Independent Living Services (SILS) and high-intensity daily personal activity supports
Advanced training or experience in complex behavioural supports or trauma-informed care
Strong understanding of person-centred support, participant choice and control
High-level communication, stakeholder engagement and interpersonal skills
Ability to coach, guide and support frontline employees
Ability to work independently, manage competing priorities and maintain service continuity
Current First Aid and CPR Certificate
NDIS Worker Screening Check
Blue Card (Working with Children Check)
Current Australian Driver Licence
Experience implementing and monitoring Positive Behaviour Support Plans
Knowledge of NDIS restrictive practice authorisation, monitoring and reporting requirements
Experience with incident management, risk assessment and reportable incidents
Experience working collaboratively with multidisciplinary clinical and allied health teams
Experience using client management and incident reporting systems
Experience managing NDIS funding requirements, including Schedules of Support and Rosters of Care
