Bayside Health is recruiting an Aged Care Assessor (Nurse or Allied Health) to conduct comprehensive home and community assessments across Melbourne's regions. The role involves travel to client homes and community sites, working within the SAS framework to determine eligibility for CHSP and related services.

The successful candidate will hold APHRA registration, have aged care assessment experience, and solid communication and planning skills.
