Care Connect, a leading not-for-profit home care provider, is seeking an experienced Duty Care Manager for a full-time maximum-term contract through to October 2027. The role supports clients, families and care teams across Victoria, NSW and Queensland, with hybrid work from Bella Vista.

You will coordinate client handovers, manage escalations and ensure safe, person-centred services when a Care Manager is unavailable. Robust collaboration and documentation are essential.
