Team Leader – Supported Independent Living (SIL)

Haven Care Australia is seeking a compassionate, organised and proactive Team Leader to lead a Supported Independent Living (SIL) team in the ACT. This is a hands‑on leadership role for someone who is passionate about delivering high‑quality, participant‑centred supports while building a positive, accountable and professional staff culture.

About the Role

As a Team Leader, you will be responsible for the day‑to‑day coordination of supports within a SIL environment. You will work closely with participants, support workers, families, Support Coordinators, allied health professionals and Haven Care's management and clinical teams.

The role combines direct participant support with operational leadership , so we are looking for someone who is comfortable leading from the front and being actively involved in the home.

Key Responsibilities

- Lead, mentor and provide day‑to‑day direction to Disability Support Workers.
- Ensure participants receive safe, respectful and person‑centred supports in accordance with their NDIS plans, goals and preferences.
- Coordinate daily operations within the SIL home.
- Monitor staff attendance, punctuality, shift handovers and completion of required documentation.
- Support effective rostering while ensuring staffing remains within approved participant funding and support ratios.
- Maintain high standards of medication management, incident reporting, progress notes and participant documentation.




- Ensure participant care plans, risk assessments, behaviour support plans and clinical instructions are followed.
- Identify and elevate changes in participants' health, behaviour, risks or support requirements.
- Liaise professionally with families, guardians, Support Coordinators, allied health professionals and internal management.
- Support staff training, induction, supervision and ongoing development.
- Facilitate team meetings and communicate changes in participant needs and organisational requirements.
- Promote participant choice, independence, community participation and quality of life.
- Ensure the home remains safe, organised, welcoming and appropriately maintained.
- Respond appropriately to incidents, emergencies and operational issues.
- Maintain compliance with NDIS Practice Standards, organisational policies and relevant ACT requirements.
- Participate in direct support shifts as required and provide leadership during complex situations.

About You

We are looking for someone who is calm under pressure, approachable, accountable and highly organised . You should be able to build positive relationships with participants while also confidently managing staff performance and maintaining professional boundaries.





Previous experience working in SIL, complex disability support, residential disability services or a similar workplace is highly desirable. Previous experience as a Team Leader, Senior Support Worker, House Supervisor or in another leadership position will be well regarded.

Essential Requirements

Successful applicants will be expected to hold or obtain the relevant employment and compliance requirements for working in disability services in the ACT, including appropriate worker screening, Working With Vulnerable People registration, First Aid/CPR and a valid driver's licence.

Relevant qualifications in Disability, Community Services, Individual Support, Nursing, Mental Health or a related field will be highly regarded.
