BlueCare is seeking a Care Partner to deliver person-centred, high-quality care across Maryborough and surrounding areas. The role focuses on care planning, coordination with clinical teams, budgets, and linking clients to resources.

You will spend much of your day on the road engaging with clients in the community. Demonstrated experience in community-based care, a Certificate IV in Health or Community Services, and a valid Australian driver’s licence are essential.
