BlueCare, part of UnitingCare Queensland, seeks a Clinical Care Partner to coordinate complex clinical and care needs across community settings in Bowen, Proserpine and surrounding areas. You will assess, plan and connect clients to multidisciplinary services to achieve optimal outcomes.
You will develop care plans with clients and teams, manage budgets, and monitor plan effectiveness while navigating care resources. Travel and field-based work are integral to the role.
