The role focuses on coordinating safe, seamless transitions from hospital to home for newborns and their families, supporting excellence in neonatal care, and ensuring best-practice discharge planning.

The successful candidate will work within the Women Youth & Children section, with relocation support for interstate candidates and flexible working
