Lead a capable Support Coordination team and give them the clarity, support and direction they need to do their best work.

We’re seeking an experienced Support Coordination Leader with strong industry knowledge and practical experience in Support Coordination. You’ll lead a team supporting people living with MS and other neurological conditions, bringing clear direction to daily operations while maintaining a strong focus on participant choice, safeguarding and service quality.

You’ll oversee caseloads, KPIs and workflows, helping the team manage competing priorities and respond confidently to complex participant circumstances. You’ll also coach and develop staff, manage performance, and set explicit expectations around quality, accountability and compliance.

As the service continues to evolve, your change management experience will be important. You’ll help the team understand and adapt to new processes, systems and NDIS requirements, while contributing to workforce planning, recruitment, budgeting, service improvements and NDIS readiness activities.

Reporting to the Support Coordination Manager, you’ll work closely with senior leaders and external stakeholders to identify issues, improve how the team operates and ensure services remain responsive to participants’ needs.

Based at our contemporary Neuro Wellness Hub on Park Road in Milton, this full-time role offers a structured hybrid arrangement: two days at the Milton Hub, two days working from home and one day at another MS Queensland location of your choice in South East Queensland each week.

This role will suit a calm and capable leader who understands Support Coordination, communicates clearly and is prepared to have constructive conversations when needed. You’ll have the opportunity to build team capability, guide the service through change and strengthen the experience of the people we support.

Are these your qualities?





- Demonstrated knowledge and experience in Support Coordination, supported by qualifications in disability or a related field, or equivalent human-services experience.

- A strong understanding of neurological conditions, the NDIS and relevant legislation.

- At least five years’ experience delivering or coordinating community-based support services within a person-centred framework.

- A relationship-builder who confidently partners with customers, families and a broad range of community services to establish strong, effective support networks.

- A skilled communicator and problem-solver with strengths in needs assessment, facilitation, negotiation and collaborative decision‑making.

- An organised self‑starter with strong planning and time management skills, experience supporting teams through change, and the ability to work independently while maintaining service quality.

- A capable team leader who can coach and develop others, set clear expectations, and manage performance and behavioural matters fairly and constructively.

- A tech‑confident professional proficient in Microsoft Office and client management systems who meets all compliance requirements.
