Capital Health Network is seeking a Services Relationship Coordinator - Primary Care in Melbourne to work within the Commissioned Services Team. You will collaborate with CHN-funded providers to oversee contracts, evaluations and relationship management to improve primary care outcomes.

You will contribute to program co-design and manage health service contracts, working with internal and external stakeholders to enhance service delivery and outcomes for ACT communities.
