About the employer:

Advantage Healthcare Support Nursing Agency is an established provider of disability and community support services. The organisation supports participants to pursue their NDIS goals and coordinates services with families, health providers and community organisations.

About the role:

The Community Support Worker assesses individual participant needs, develops and reviews tailored support plans, and coordinates health, community and NDIS-funded services. The role is a planning and coordination position. It is not a personal-care or disability support worker role. The position is based at Dee Why and requires travel to meet participants, families and service providers.

Key responsibilities:

- Assess participant needs and develop, implement and review individualised support plans with participants, families and carers.
- Coordinate appropriate health, community and NDIS-funded services and supports, and liaise with external providers.
- Support participants to build independence and work towards the goals in their support plans.
- Monitor progress, wellbeing and changes in support needs, and **escalate** concerns, incidents and hazards.
- Work with families, carers,



case managers and healthcare professionals.
- Maintain accurate client notes, progress reports and service records.
- Participate in case reviews, team meetings and training.

Skills and experience required:

- A qualification at diploma level or higher in a closely related field.
- Demonstrated at least 1 year full time experience in relevant work.
- Solid written and verbal communication skills, including accurate case notes and reports.
- Ability to work independently and with families, carers and external providers.
- A current full Australian driver's licence and willingness to travel across the Northern Beaches.

Other requirements:

- NDIS Worker Screening Check.
- Completion of the NDIS worker orientation module.
- Working with Children Check.
- Current First Aid and CPR certificate.
- Comprehensive car insurance if using a private vehicle for work travel.
