Lumia Care Group is seeking an experienced Care Coordinator in Adelaide to support clients in a hands-on role. You will coordinate Support at Home services, liaise with Care Partners and frontline teams, and ensure services meet funding, compliance and quality requirements.

The successful candidate will have hands-on Support at Home or CHSP experience, strong CHSP funding knowledge, and a commitment to person-centred care. This full time role is based in Adelaide.
