About the role

Working within a multi-disciplinary team environment (telehealth / face to face) this position focuses on the provision of government funded carer support services that are responsive to the needs of the local community. This position encompasses a variety of roles including information provision; guided referrals; facilitation of peer support groups; community outreach; developing carer capacity and resilience; delivering practical and innovative practical support.

Key Responsibilities

- Undertake holistic assessments of clients' need and develop goal-directed care plans
- Respond to emergency, practical and emotional needs of carers
- Coordinate service delivery and refer to other service types as required
- Contribute skills, knowledge and expertise to promote community awareness and understanding of carer issues though outreach and promotional activities
- Providing carer coaching and support to carers to reach their goals

What we are looking for

- Relevant qualification in Social Work, Welfare, Aged Care, Mental Health, Disability or Community Case Management, with an understanding of the needs of carers and the people they support.
- Excellent communication and interpersonal skills,



with the ability to build effective relationships and work collaboratively as part of a team or independently.
- Strong organisational and digital skills, including the ability to manage competing priorities and use Microsoft Office, databases and content management systems effectively.
- Commitment to continuous learning and quality care, with public health sector experience and consumer group facilitation skills highly regarded.
