UnitingCare Children, Young People and Families is seeking a Clinical Care Partner to oversee and coordinate complex clinical and care needs in the community across New Farm, Hamilton, Kedron, McDowall and surrounding areas in Queensland. You will develop and review complex care plans with clients and the multidisciplinary team, manage budgets, and ensure high-quality, person-centred care.
